Diabetes is a chronic and often progressive disease. Patients are initially diagnosed and managed by health care teams but despite appropriate care many patients experience complications such as diabetic retinopathy, diabetes induced renal disease, diabetic neuropathy, and diabetic vascular disease. Unfortunately, the electronic medical records and specifically the patients ICD-10 problem list may not reflect the added complications. If the medical chart is deficient in cataloguing these complications, this can have ramifications for the health care teams when it comes to reimbursement especially in the newer payment models.a. What You Will Learn During this internship, a student will have the opportunity to: -Perform data analysis of electronic medical records to identify chronic disease patients whose problem lists do not accurately reflect their disease state -Develop a feedback loop to provider teams for correction of problem lists -Work with provider leadership to develop potential strategies to improve diagnosis coding -Shadow providers or other staff to learn how current workflows impact coding accuracyb.
